Differentiated Service Delivery Model for Management of Non-Communicable Diseases

Since 2021, MSF has been providing decentralized care for chronic diseases in Homa Bay County at two primary healthcare facilities in Nyalkinyi and Marindi. Kenya is experiencing an epidemiological transition in disease burden with the rapidly increasing burden of noncommunicable diseases (NCDs) which account for up 39% of deaths in the country. The three major NCDs: Cancers, Diabetes and Chronic Respiratory Diseases comprise 57% of all NCD deaths in Kenya.

MSF in Homa Bay has pioneered a differentiated service delivery model for management of Diabetes and Hypertension to be responsive to patient needs. This communications initiative focuses on showcasing the differentiated service delivery model and a focus on the decentralized sites and the need of chronic disease care at primary health care facilities.
