For the children and caregivers: Written consent was obtained on the day of the pre-arranged interview, except for Gertrude Dennis who cannot read or write, so she provided a fingerprint.

The cut-off slip that should be retained by the consent-giver was returned to the families by the psychosocial team in a follow-up visit.
