MSF started a malnutrition project in March 2021, after several alerts regarding the nutritional situation in Southern Madagascar.
Teams are currently operating in Ambovombe and Amboasary districts, with medical projects (support to Ambovombe pediatric hospital, mobile clinics), water and sanitation projects (drilling, water network and hand pumps rehabilitation, water trucking) and food distributions. Between March and December 2021, more than 11,000 children under the age of 10 were treated thanks to MSF mobile clinics.

On a vu, il y a trois semaines, un jeune garçon de 13 ans arriver avec sa grand-mère. Il tenait à peine debout, il était à quelques kilomètres seulement de la clinique mobile. 13 ans, 1,45 m et 23 kg si je ne me trompe pas.

La période de soudure, c'est cette période entre deux récoltes durant laquelle les populations ont épuisé les stocks récoltés lors des récoltes précédentes et n'ont pas encore récolté les prochaines. Ils sont dans une situation où il y a très peu, voire pas du tout de nourriture disponible. C'est un moment qui est particulièrement important en termes de nutrition puisque c'est une dégradation progressive jusqu'au mois de mars de l'état des enfants.

Pour pallier le manque de nourriture,
MSF a repris ses distributions dans le sud de Madagascar

Le nombre de personnes vulnérables qui sont touchées à Madagascar est très, très, très important. Les enfants restent très longtemps dans les cohortes et ne prennent pas suffisamment de poids, ce qui est médicalement logique s’ils reçoivent simplement les intrants nutritionnels thérapeutiques, c'est insuffisant.

d’approvisionnement en eau, hygiène et assainissement

Il y a quelque chose d’assez simple quand on arrive dans une zone, quand on commence à parler avec les personnes et qu'on leur demande de quoi elles ont besoin, 100% du temps, les personnes nous répondent : de l'eau. Ce n’est jamais facile de s’imaginer mais je pense qu'un point important, c'est de réaliser que les personnes marchent quatre, cinq, six, dix voire 20 heures pour pouvoir récupérer un bidon d'eau. Donc il y a un vrai problème de disponibilité de l'eau à Madagascar et c'est une activité cruciale.

Et puis je pense à un autre exemple qui m’a beaucoup marquée en lien avec l’indisponibilité d’eau au quotidien. C'est un bébé que nous avons reçu sur la clinique mobile d'Ambatomainty, dans le district d'Amboasary. Huit mois, 4,6 kg et quand on essayait de faire le test d'appétit avec le plumpy'nut, la pâte d'arachide, le bébé ne mangeait pas. On s'inquiétait énormément, et finalement, quand l'infirmière a eu l'idée de lui faire boire de l'eau, il a bu une tasse et après, il a mangé son plumpy'nut. Et ça, pour moi, c'est particulièrement choquant, des enfants qui pourtant ont faim, très faim, mais ils ont tellement soif qu'ils ont plus soif que faim.

La problématique de la malnutrition de façon prolongée comme c'est le cas à Madagascar, avec en plus ces épisodes aigus, ce sont tous les effets sur le développement : le développement physique, le développement psychomoteur, le développement d'un nombre assez important de fonctions du corps, le corps qui est naturellement obligé de se mettre en économie. Cette économie implique des retards de croissance, la croissance au sens large du terme, pas simplement se dire : ce sont des enfants qui sont trop petits ou trop maigres.

Three weeks ago we saw a 13-year old boy arrive with his grandmother. He could barely stand up. He was only a few kilometres from the mobile clinic. I think I’m right in saying he was 13, 4 foot 7 and that he weighed 23 kilos.

The hungry season is the period between two harvests when people run out of food stocks from the previous harvest and must wait for the next one. So they find themselves in a situation where they have very, very little or no food at all. It’s a particularly challenging time for nutrition as children’s health steadily worsens until the following March. It’s absolutely vital during this period to ensure that as many vulnerable people as possible have access to food distributions.

The number of vulnerable people who’re affected in Madagascar is extremely high. Children stay sick for a really long time. They don’t gain enough weight, which, from the medical perspective, is logical. If all they’re getting is therapeutic food, it’s not sufficient.

water, hygiene and sanitation

When we arrive somewhere and start talking to people and asking them what they need, 100% of the time they answer water. It’s not easy to picture, but it’s important to be aware that people can walk as much as four, five, six, ten or even 20 hours to get one jerrycan of water. Water is a major problem in Madagascar, so ensuring its supply is imperative. We sometimes see children arriving in our mobile clinics who, even though they’re suffering with moderate or even severe malnutrition, are incapable of doing appetite tests or eating food we give them because they’re so thirsty.

I have another example related to the water shortage I can’t get out of my mind. It was a baby we examined in the mobile clinic in Ambatomainty in Amboasary district. She was 8 months old and weighed 4.6 kilos. We tried to do the appetite test using a peanut paste called Plumpy'nut®. The baby wouldn’t eat and we were really worried. Then at last the nurse came up with the idea of getting her to drink water. She drank a cup-full and then ate some Plumpy'nut®. I find it profoundly shocking that children who’re starving, really starving, are so desperately thirsty that their thirst is even worse than their hunger.

The problem with long-term malnutrition like we see in Madagascar, and which is exacerbated by acute episodes, are the many effects on children’s development, from physical and psychomotor through to numerous bodily functions. The body is automatically forced into economy mode, and this economy leads to stunting, but also its growth in general. We mustn’t think that these kids are simply too small or too thin.
