02:05: By decentralizing….you manage to move the diagnosis and the treatment on management from the specialised care to a general practice and to a nurse level model of care.
03:19: It benefits the patients in many ways….They (Patients) can have access, closer to their homes, closer to the area they live in. Imagine this a treatment that you need to monitor, you need to have a follow up consultations, you need to travel quite a lot of time for follow-ups… Patients need to pay their transports, they need to use a lot of time to go to the hospital. and

2. What lessons did we learn in the elimination of Hep C in Meerut, India?

04:15: When you need to decentralize, you have to simplify.
10:19: Still, more work needs to be done. More accessible diagnostic tools and faster, so the point of care tools need to be developed and In terms of treatment, when we started HepC, it was super expensive and very toxic, very low tolerance. Only 3 out of 10 patients managed to complete the treatment because of adverse effects and cost. And today, the treatment is very simplify, it's more tolerable, more accessible but still, extremely expensive.
11:09 So, cost for drugs is one of the key issues that globally has to be addressed to be able to really reduce mortality related to HepC.

3. What role can patients and communities play in the fight against Hep C spread?

05:43: When you work on HepC, prevention is critical. Communities are accountable…..
06:56: In our case, we did manage to work with patients to empower them to be the voice of the no! reused syringe 11:36: And communities are responsible for health of the community. Not only the beneficiaries, and they are the one that can carry on the prevention messages. They are the ones who carry on the follow-up of patients…..There is a social network, at community that is allowing patients to feel embraced, supported and as well finalise the treatment... Communities are at the center, it's where we need to read what they need.
