To help with the coronavirus pandemic in France, Médecins Sans Frontières has rolled out a threefold strategy: providing support to vulnerable groups, to medical facilities, health centres and hospitals, and to care homes.

#1 providing support to vulnerable groups

We have mobile teams reaching out to people either living in extremely tough conditions in makeshift camps and squats or in shelters. The teams offer them medical examinations and testing for coronavirus. When a person tests positive for the disease, but doesn’t need to be admitted to hospital, we take them to coronavirus treatment centres managed by other organisations. In some centres, Médecins Sans Frontières is in charge of providing medical treatment. People in these centres have somewhere to live while they’re under medical supervision.

#2 providing support to medical facilities

Very rapidly, we made contact with hospitals and health centres across France, which resulted in partnerships with a number of facilities. One is Henri Mondor Hospital. We’ve helped with downstream beds where patients released from intensive care continue their treatment. We also work with the university hospital in Reims. As well as working with them on restructuring, we installed a tent facility to enable the hospital to accommodate more intensive care beds.

In Marseille, we’ve initiated a partnership with two health centres in the city’s northern suburbs. We have increased their capacity in terms of outpatient consultations, triage, coronavirus testing and referrals. In these districts the average number of people living in one household is extremely high—I think it’s eight to ten people for each apartment.

#3 providing support to care homes

The third element has been deploying mobile teams to provide support to care homes. We have teams going to a number of homes where they help with reorganising their way of working, and also ensure the hygiene measures are followed as far as is possible. We sometimes give medical consultations, because many residents no longer have access to their own doctors. And lastly, we offer psychological support, particularly to staff.

La stratégie de Médecins Sans Frontières face à l’arrivée de la pandémie de Covid-19 en France se met en place à travers trois volets: l’appui aux populations précaires, l’appui aux structures de santé, centres de soins ou hôpitaux, et l’appui aux EHPAD.

#1 appui aux populations précaires

Ce sont des équipes mobiles qui se projettent soit auprès de campements informels, ou de squats, où vivent des personnes en grand précarité, ou alors auprès d’hébergements collectifs pour leur proposer des services de consultation médicale et de screening, de dépistage par rapport au Covid. Et dans le cas où les personnes sont positives et ont la maladie, mais qu’elles n’ont pas besoin d’être hospitalisées, et bien on les emmène dans ce qu’on a appelé des centres de desserrement, des centres Covid+ qui sont gérés par d’autres associations, et pour certaines d’entres elles, c’est Médecins Sans Frontières qui s’occupe de la santé. Et donc elles sont là, logées et suivies médicalement, surveillées.

#2 appui aux structures de santé

Ensuite, très vite, nous avons été aussi en relation avec des hôpitaux, des centres de santé en France. Cela a débouché sur plusieurs collaborations, en particulier au CHU de Mondor pour des fins d’hospitalisation, après la période en soins intensifs. Cela a été le cas avec le CHU de Reims, ou là nous avons à la fois réfléchi avec eux sur la réorganisation et aussi installé une unité dans laquelle le CHU de Reims a pu installer des lits de réanimation.

A Marseille, nous avons initié une collaboration avec deux centres de santé, en lien avec l’hôpital Nord de Marseille, il se trouve dans les quartiers nord. Nous avons renforcé leur capacité de consultations externes, de triage, de dépistage du Covid et de référence. On parle de quartiers où la moyenne de personnes qui vivent dans un même foyer est très élevée, je crois qu’elle est entre huit et dix personnes par appartement.

#3 appui aux EHPAD

Le troisième volet a été la mise en place d’équipes mobiles là aussi de soutien à des EHPAD. Nous avons mis à disposition des équipes qui se rendent dans certains de ces EHPAD et qui travaillent avec elles à la meilleure manière de se réorganiser là aussi, d’essayer de respecter au mieux les précautions d’hygiène. Nous sommes aussi amenés à faire des consultations médicales parce que beaucoup des résidents qui étaient suivis par un médecin ne le sont plus. Enfin, nous proposons un soutien psychologique, au personnel en particulier
